Parent and Child Participation Agreement

This Liability Waiver and Photo Release ("Agreement") is entered into between Hands-On Home Skills with Brittany ("the Business"), located at 19516 N Rose Ct, Maricopa, AZ 85138, and the undersigned Parent/Guardian ("Parent") and Child Participant ("Child") in connection with the participation in hands-on home skills lessons provided by the Business. By signing this Agreement, the Parent and Child acknowledge and agree to the following terms and conditions:

 

  1. Acknowledgment of Risk:

I, the Parent/Guardian of the Child, understand and acknowledge that there are inherent risks involved in participating in hands-on home skills lessons, which may include, but are not limited to, physical injury, burns, cuts, allergic reactions, and other risks associated with cooking, using tools, materials, and equipment during the lessons. I acknowledge that these risks exist and voluntarily accept them.

 

  1. Assumption of Risk:

I, as the Parent/Guardian, voluntarily assume all risks associated with the participation of my Child in these lessons, including, but not limited to, the risks mentioned above. I agree that the Business and its instructors are not responsible for any injury, loss, or damage that may occur as a result of my Child’s participation, except in cases of gross negligence or intentional misconduct.

 

  1. Waiver and Release of Liability:

I, for myself, my Child, and my heirs, executors, and administrators, hereby fully release, waive, discharge, and hold harmless Hands-On Home Skills with Brittany and its owners, instructors, employees, contractors, agents, and affiliates from any and all claims, demands, causes of action, liabilities, or damages arising from or related to my Child’s participation in the lessons, whether caused by negligence or otherwise.

 

  1. Medical Emergency:

In the event of a medical emergency, I consent to the Business seeking emergency medical treatment for my Child, if necessary. I also agree to be responsible for any medical costs or expenses incurred as a result of such treatment.

 

  1. Parental Responsibility:

I affirm that I am the legal parent or guardian of the Child listed below, and I have the authority to sign this waiver on behalf of my Child. I agree to ensure that my Child complies with all rules, guidelines, and instructions during the lessons.

 

  1. Permission to Use Photographs and Video:

I, the Parent/Guardian, hereby grant permission to Hands-On Home Skills with Brittany to take photographs, videos, and other media of my Child during the lessons. I understand that these images and videos may be used for promotional, educational, and marketing purposes on websites, social media platforms, printed materials, or other public-facing media, without compensation or further approval required.

I agree that these photos/videos may be used in a manner that promotes the Business, and I waive any right to inspect or approve the final product.

 

  1. Indemnification:

I agree to indemnify and hold harmless Hands-On Home Skills with Brittany and its instructors, employees, agents, and contractors from any claims, lawsuits, damages, or expenses (including attorney fees) arising from my Child’s participation in the lessons or the use of photographs and videos.

 

  1. Governing Law:

This Agreement shall be governed by and construed in accordance with the laws of the state of Arizona, without regard to its conflict of law principles.

 

  1. Severability:

If any provision of this Agreement is found to be invalid or unenforceable, the remainder of the provisions shall continue in full force and effect.

 

  1. Acknowledgment of Understanding:

I, the undersigned, have read, understood, and agree to the terms and conditions outlined in this Liability Waiver and Photo Release Form. I am aware that by signing this form, I am waiving certain legal rights, including the right to sue the Business for any injuries or damages that may occur during my Child’s participation in the lessons or the use of photos/videos.

By electronically completing the form below, I acknowledge that I have read, understand, and agree to the terms and conditions of this Agreement.
